Please complete the form below to register for the 2020 Henry Cluster STEMM Foundation Mini Golf Challenge. Each adult team mentor and student team member must complete a separate registration. Complete registration includes acknowledging and accepting the terms of the 2020 Mini Golf Participant Agreement and Release of Liability statement.

2020 Mini Golf Challenge Participant Agreement and Release of Liability
1. Neither The Henry Cluster STEMM Foundation nor Mission Trails Church maintain health insurance for injuries to the participant that may arise out of involvement in this activity. *
2. By virtue of participation, PARTICIPANTS RISK BODILY INJURY INCLUDING, BUT NOT LIMITED TO, PARALYSIS, DISMEMBERMENT, DEATH AND OTHER LOSS. *
3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISK FOR MYSELF OR MY CHILD *
4. I do hereby waive, release and forever discharge The Henry Cluster STEMM Foundation, Mission Trails Church and their officers, agents, employees, representatives, executors, and all other related parties from any and all responsibilities or liability from injuries or damages resulting from participation in any activities for this event. I also hereby release all of those mentioned, and any others acting upon their behalf, from any responsibility or liability for any injury or damage to participant, including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf, or in any way arising out of or connected with participation in any activities for this event. *
5. I understand and am aware that using construction material to build a golf hole is a potentially hazardous activity. I also understand that this activity involves a risk of injury and even death, and that the participant is voluntarily participating in these activities and using equipment with knowledge of the dangers involved. I hereby agree to expressly assume and accept any and all risks of injury or death to the participant. *
6. This Agreement is binding on my heirs, personal representatives, next of kin, spouse and assigns. *
7. I certify to the best of my knowledge that the participant’s current physical condition is satisfactory for participation in this activity and that participant is free of any health issues that would affect participant’s ability to participate. *
